IMANI Foundation, Inc. services are flexible, community based and client oriented. Assessments are conducted to identify treatment and support needs. Community referrals and linkages are arranged to professional agencies, community programs and other resources.
The philosophy of IMANI Foundation, Inc. is to insure the timeliness and appropriateness of referrals. Each ease manager will work with the referring provider to ensure the smooth transition of the client into treatment and into case management support services.
The strength based Case Management approach assumes those clients with complex and multiple needs will access services from a range of service providers and the goal is to achieve quality service delivery.
